RULES

  • Only EXISTING PATIENTS are eligible for the contest.
  • Existing patients must have their referral submit an online appointment booking request (https://bellscornersdentistry.com/book-appointment/) and specify the referrer’s name.
  • Referrals must book AND attend their appointment for both the referrer and referral to be entered into their respective draws.
  • Referrers are not eligible for contest if referrals do not attend their scheduled appointment.
  • Emergency appointments are not eligible for contest.
